Under Your Doctor's Supervision All of our home medical care and services are performed under the guidance and supervision of your very own physician. The Home Health staff works closely with your doctor, notifying him or her of any change in your condition and sending written reports on a regular basis.

How Do You Qualify?
Home Health patients must be under the supervision of a licensed physician and have a safe home environment that allows for effective, intermittent healthcare. You or a loved one can be referred to Home Health through physicians, nurses, social agencies, hospitals or friends and family. If you are currently hospitalized, we will work with your social worker or discharge planner to ensure a comfortable transition from hospital to your home.
